Your
AutoCAD has a S::STARTUP routine that is defined using (defun
S::Startup() ( your stuffs here ) ) You
can simply fix this error by adding a -q
after (defun, as shown below (defun-q
S::Startup() ( your stuffs here ) ) Use
Windows search to search your AutoCAD and customization folder for *.MNL, *.LSP files that
contain the string S::STARTUP.
If you
install your AutoCAD in the default folder, most of the time your will
find S::STARTUP in these files.

find the *.LSP or *.MNL file containing S::STARTUP, edit it using
any text editor, change (defun
S::Startup() to
(defun-q
S::Startup()
by inserting a "-q", save the change.
Re-start
AutoCAD and you will not get this error any more, and, CADTool will
function properly.

FAQ 2-3:
MicroStation version: CADTool menu is not loaded
Follow these steps to complete Office2CAD installation
Step 1
In MicroStation, run key-in MDL Load C:\CADTool\USTN\Setup

Step 2
You will see a installation success dialog box, and the CADTool menu will appear at the end of your menubar

Step 3
The installation is completed.

Please note you have to run the same installation if you work in a different workspace.
